The Pennsylvania Department of Health has changed the school immunization regulations beginning in August 2017. Please note the following information regarding attendance in the Hopewell Area School District.
Starting with the 2017-2018 school year, students will risk exclusion (unable to attend school) until the District has a record verifying that they have received all of the required immunizations.
This complete immunization record MUST be submitted to your child’s school by August 27, 2018. Please plan accordingly so that you have time to see your child’s physician for the necessary immunizations.
You can drop off of your child’s health records in the District Office (2354 Brodhead Road) or deliver or mail them to your child’s assigned school. You can also scan and email the records to your child’s school nurse (contact information below). If your child needs any required immunizations, please call your child’s physician's office to share this information with them and schedule the appropriate appointment. If you are underinsured and have concerns with the cost, the Pennsylvania Department of Health in Beaver County offers assistance. Please call them at 724-774-1385 for more information. Students in ALL grades (K-12) need to following immunizations for attendance: 4 doses of – DTaP (diphtheria, tetanus, and acellular pertussis - 1 dose on or after 4th birthday); 4 doses of Polio (4th dose on or after 4th birthday and at least 6 months after previous dose given); 2 doses of - MMR (measles, mumps, rubella); 3 doses of - Hepatitis B; 2 doses of - Varicella (chickenpox) vaccine or evidence of immunity. Seventh (7th) Grade ADDITIONAL immunization requirements for entry: 1 dose Meningococcal Conjugate Vaccine (MCV) 1 dose of Tetanus, diphtheria, acellular pertussis (Tdap) Twelfth (12th) Grade ADDITIONAL immunization requirement for entry: 2nd dose Meningococcal Conjugate Vaccine (MCV) must be at 16 years of age or older. (If the first MCV was given at age 16, then they only need one) PLEASE NOTE: The only exemptions to the school laws for immunizations are: medical reasons, religious beliefs, philosophical/strong moral or ethical conviction. Statements of exemption must be provided in writing. If your child is exempt from immunizations, he or she may be excluded from school during an outbreak. |
