Use the links below to download all of the necessary forms for the dance. Please use the following information in determining what documents are necessary.
- EVERY attendee currently attending school (or of school age) MUST have a completed and signed "Student Parent Permission Slip"
- ALL attendees currently attending a high school other than Hopewell MUST have a completed and signed "Principal Sign-Off Page"
- ALL attendees who have graduated MUST have a completed and signed "Graduated Guest Agreement"
