Any student (7th through 12th grade) wishing to participate in a 2024 Fall sport for Hopewell Area School District must have a physical examination prior to the start of official practice which is Monday, August 12, 2024, except Golf must have their Physicals submitted by Monday, August 5, 2024. Instructions and the PIAA Physical Form are available under “ATHLETIC PHYSICALS” on the left side of this page. Sections 1-5 need to be filled out by a parent or guardian and Section 6 needs to be completed by the physician.

PLEASE HAND IN THE PHYSICALS AT EITHER THE FRONT OFFICE OF THE JR. HIGH OR HIGH SCHOOL, OR THE ATHLETIC OFFICE.
